Pushing a cart
What happened
on [REDACTED] a Welder was pushing a cart through the double swinging doors when they tripped and fell roughly 3 feet forward and later experienced pain in their hip lower back left shoulder and left wrist. The incident was reported to their manager within just a minute or two. During the debrief the EE stated they were doing alright and from there it would continue as a near miss opportunity. Roughly 10 - 15 minutes after the initial report the EE returned to state that they were starting to experience discomfort and tightness and wanted to go home and rest. The EE described the incident as follows: On their way out of the room they paused for a moment to let someone by who ended up waiting to let the EE through with the cart. Once EE moved forward again the swinging door began to close and their right foot got caught under the cart bumpers attached to the door. This resulted in the EE tripping and attempting to utilize the cart to regain balance. EE stated they are unsure whether or
