Employee was attempting to attach a bell clamp that fell off [REDACTED].

He was in a rush and was trying to tighten it before the pipe indexed over to the mandrel. As a result the clamp engaged causing it to close around the mandrel while the employee 's left hand was caught in between the clamp and the pipe on the mandrel being belled causing a laceration to the employee 's left-hand palm.
