There was a hose in the tank already and cleaning was on-going when [REDACTED] opened the tank and the hose inside pushed forward to the direction of her face. [REDACTED] reached to hold the hose with her left hand and the holding tank lid fell on her left hand. [REDACTED] was not holding the lid down with her right hand when the lid fell on her left hand.
