Employee was coming out from one of the blocks from [REDACTED] carrying his ladder to place in the trailer. [REDACTED] stated he wasn 't paying attention and he stepped in a hole in the ground causing him to loose his balance. This caused the ladder to slip from his hands and it was going to strike his right shoulder and the right side of his face but he placed his right hand in front of him shielding himself.
