Before the incident
Associate was called to the Custom Cut machine for complaint from operations that the Load Dump roll was making a popping noise. He locked out the machine to enter the roll entry area to troubleshoot the issue. The drive chain was found to be loose which was causing the chain to jump when commanded to rotate the roll assembly. When associate disconnected the chain at the master link he was not aware that the chain supports the assembly.
What happened
The roll then rotated downwards striking him in the left arm.

Associate was taken to the Salem ER where it was x-rayed and determined to have a small fracture in the inside bone near his left wrist.
