What happened
Employee was changing blades to the shredder. He reached under to remove a bolt with his left hand to hold onto the bottom of the bolt and with his right hand he was using an impact wrench to remove the nut. The bolt spun while holding the rounded portion of the bolt and the sharp worn edge of the bolt cut his finger.
