What happened
Employee attempted to enter the runout conveyor to clear a jam. He walked around his job station to the stairs that go up and over the runout channel. He entered the conveyor area through a safety gate that was not working at the time. The safety latch was not attached properly allowing entry without requiring the equipment to be isolated. Employee neglected to turn the switch that shuts off the line before entering the machine. While the machine was running Employee stepped on a platform with his left foot. His foot was caught between a rotating auger and the platform.
