Before the incident
Employee was inspecting the chains and sprockets on the preforms of CP 25-36. Employee removed the covers on the drivers of CP 33 preform and while the sprockets and chains were motorized he checked the tightness with a gloved finger

The tip of the glove was caught in the chain and the tip of the finger was pinched in the sprocket causing amputation of the tip of the right middle finger
