What happened
The employee was trained and used the process many times. He was supposed to slide the square lid diagonally over the open vault so it would not catch the vault rim lips and then rotating it so the lid would fall into place. During the lid rotation the vault suddenly fell on his finger and his finger was in between the vault lid and the vault rim.
