What happened
while employee was preheating the machine he dropped the cap to the adhesive used for preheating. He bend down to pick up the cap not realizing he was in the machine 's danger zone. A platten on the machine struck the employee in the head which caused him to strike his face on the plat in front of him lacerating both his head and face.
