Before the incident
Retrieving tail of coil that had passed through entry stitch section.
What happened
Associate was on phone with metallurgical lab getting metal testing results. While on phone coil finished processing through entry end. Being distracted by phone call operator did not stop entry section in time to allowing normal processing of connecting one coil to the next. Entry section was stopped with tail of metal between exit pinch roll section and #1 bridle. Operator opened exit pinch rolls (normal process is rolls close automatically when entry end of machine is stopped) then opened access door to section where tail was located. At this time entry end operator of #6 strip anneal arrived to assist. Rolls had at this point closed on their own and #6 strip anneal operator opened them back up. Machine operator retrieved tail of coil and inserted it back in to exit pinch roll section. Once tail of metal was through exit pinch rolls aiding operator stopped depressing exit pinch roll up button. Machine operator was still feeding strip through exit pinch rolls.
