Before the incident
We were conducting production trials on Line 126 The sheet came out of the rails and jammed at the oven and former The scrap sheet was fed to the grinder after the jam The employee running the grinder controls fed the sheet into the grinder at too high of a rate This resulted in a grinder jam A Maint Technician was utilizing a 62 inch wooden 2 x 4 to rotate the rotor and remove the plastic sheet from the grinder The technicians hand was at the end of the 2 x 4 providing leverage to move the 2 x 4 in an up down motion The technicians hand was pinched between the 2 x 4 and grinder pit wall This resulted in a hand laceration The technician was wearing Level 4 cut resistant gloves but they do not offer much impact protection
