Before the incident
Operator was assisting with clearing a slip sheet jam on the dunnage rollers. As the dunnage pallets and slipsheets were discharging toward the dock door the top portion of the slipsheets fell off the pallet and became lodged in between the rollers. The pallets and remaining slipsheets continued to the dock door while leaving behind a large amount of slip sheets lodged.
What happened
At this point the operator used shutdown the equipment and entered depal to clear jam. As the operator applied force and the slip sheets became dislodged the residual force caused the operators hand and slipsheets to come into contact with their chin and lower lip resulting in lacerations.
