Before the incident
While trying to remove the seamer platform (prepping for [REDACTED]) EE was attempting to secure a portion of the platform to the fork truck mast. Platform had already been moved from it 's original position and was resting on a fork truck bumper.

While re-securing fork truck mast to platform the platform slipped off the fork truck bumper and a post of the platform landed on EE 's foot.
