Several billets had jammed up on the conveyor and a forger trainee went over to break loose the jam. He used a pry bar holding it at the flat angled end and used the narrow end to poke the billets in an effort to free them.

The narrow end of the pry bar slid down the billet in between the conveyor rollers which were still in motion moving towards the right. This was moving the pry bar and his hand to the right where his right hand got wedged between the moving pry bar and the stationary upper conveyor hood. As the conveyor continued to move it pushed the pry bar through the employee’s right hand.
