Before the incident
EE was picking up a tire to load it into bed of pickup truck and injured his back. [REDACTED] was notified that day and had him on a first aid treatment plan. He reported worsening symptoms on [REDACTED] follow up call with [REDACTED] and they kept him on first aid plan. On [REDACTED] [REDACTED] 13 EE went to urgent care on his own claiming the pain got much worse overnight. He didn t notify [REDACTED] prior to going to the clinic. The clinic told him to take off work from 7 15 7 17.
