What happened
Tooling is stored approximately 30 feet from machine. The cutter head was placed onto cart and then pushed to machine. When tooling is set on its end it creates a very unstable position while the cart is moving. This is what led to the cutter head rolling off the cart and hitting worker’s leg causing a laceration.
