IW WAS CALLED TO THE HEEL BUILDING TO SERVICE A PIECE OF FINISHING EQUIPMENT
What happened
IW WAS WORKING ON THE FINISHING EQUIPMENT AND HE KNELT DOWN ON A PIECE OF CARDBOARD THAT WAS ON THE FLOOR UNDER THE MACHINE. THE CARDBOARD HAD BEEN CONTAMINATED WITH THE FINISHING CHEMICAL AND THE CHEMICAL PENETRATED THE IW PANTS AND THE SKIN ON HIS KNEE WAS EXPOSED TO THE CHEMICAL.
