[REDACTED] was inside the forklift with his seat-belt on loading pallets on to a truck at Door # 4. [REDACTED] stated he was loading the truck and the truck pulled out while he was between the tail of the truck and the dock leveler plate.

[REDACTED] was loading the truck and the truck pulled out while he was between the tail of the truck and the dock leveler plate. He then fell through the gap slamming backwards on the forklift on to the asphalt. [REDACTED] was out doing his first delivery run for the day. [REDACTED] texted him that he was going to do Harvest Bev for his second run while on his way back from doing his first set of deliveries. [REDACTED] gets back to the shop and backs into DOOR 4 per [REDACTED]s instructions. [REDACTED] completed his paperwork (return and short sheet) and he let the warehouse know that he had a pickup of 6 cans of coffee on the truck.Then [REDACTED] let [REDACTED] know that there were a lot of pallets and that the delivery may be two separate runs. Next [REDACTED] went to talk to [REDACTED] about PJ’s produce. They were short their yellow peppers. He then returned to his truck to wait for thewarehouse to load his truck with the Harvest Bev order. [REDACTED] [REDACTED] walke

Joni was inside the forklift with his seat-belt on when he fell from the inside of Door # 4 's dock plate backwards slamming onto the outside asphalt.
