Before the incident
Forklift operator was unloading the trailer the wrong way & was in a hurry. Workers removed the straps from first of 3 layers of bundles that were stacked. [REDACTED] unstrapped the second layer of a bundle. [REDACTED] was instructed to remove the other straps from the load.
What happened
Forklift operator lifted two of the bundles. Forklift driver punched the bundle that was in the middle into [REDACTED] while trying to remove the last bundle from the trailer. The bundle rolled down to where [REDACTED] was standing & knocked him down folding in half.
