Before the incident
On [REDACTED] at approximately [REDACTED] [REDACTED] delivery driver who was working at the [REDACTED] facility reported an injury on [REDACTED] to his supervisor Driver stated on [REDACTED] while delivering product to a customer he was stacking cases on his hand truck when he turned to put the case on the hand truck As the driver was turning he felt something pull in his right lower back area driver continued to complete his route for the day The driver did not report this incident until [REDACTED] when he was already back in [REDACTED] on a route which he reported to his supervisor Driver stated he did not report the incident as he thought it would get better however it did not Fleet Safety Manager [REDACTED] then spoke to driver and sent him to the Concentra Clinic in [REDACTED] All documents are attached to this report from Concentra
