Incident occurred around [REDACTED] [REDACTED] was working pick areas 4345 of the bottle line This pick area has shelves that are 4 levels Step ladders are supplied if needed to reach the top shelve hanging at end of the aisle The incident wasnt caught on any of the security cameras There were no witnesses According to the employee statement Climbing up onto the tops shelf to grab 3 bottles when coming down the shelf my ankle twisted to the side as was stepping down and I fell As I fell my foot hit the ground sideways and I then dropped the bottles I had in my hands as I landed on the floor The bottles landed on top of me and I stayed on the floor for a few seconds Got up and limped to the front to get a manager According to Supervisor on duty [REDACTED] [REDACTED] did not have on proper footwear when he came to investigate There is also a discrepancy on which ankle was twisted [REDACTED] reported the right ankle but when [REDACTED] gave [REDACTED] an icepack [REDACTED] placed it on he
