On [REDACTED] Transportation Supervisor [REDACTED] sent an email advising SGWS Driver called him to report he was on the way to the doctor for an injury The driver reported the injury occurred on [REDACTED] at customer location Tobacco Plus [REDACTED] Opelousas between [REDACTED] while he was moving cases he felt a pop in his right knee and has been hurting since Between the reported injury time and time of doctor visit driver never complained about being injured while working Driver came in [REDACTED] and just stated his knee was hurting to supervisor [REDACTED] never stating to [REDACTED] he was injured from a work incident and only asked if he could have a helper assigned to him Driver also did same on [REDACTED] and advised he was going to the doctor after work Supervisor was not notified anymore until [REDACTED] on [REDACTED] when driver stated he was going get an xray Driver was off [REDACTED] came to work on [REDACTED] making the original complaint of his knee was sore The super
