Before the incident
Employee was grabbing 4x4x10 ' pieces of lumber out of the wood rack to utilize for steel plate dunnage.
What happened
The rack is constructed of 4ea 2x2 square tubing that act as the uprights (posts) that are roughly 3 ' from one another on all four sides and approximately 3 ' in height with lower supports attached between each post. Employee grabbed one 4x4 and set it across the top of two of the posts. He then bent over that 4x4 in attempt to grab a second 4x4 so he ultimately could carry two pieces at the same time. While bent over and grabbing the second 4x4 his mid torso bumped the 4x4 resting on the posts causing it to fall back into the rack crushing his thumb against the piece of lumber he was lifting out.
