What happened
Employee was putting away a stack of unassembled boxes from the dock. to aisle #2 they came in 2 bundles not on a pallet one atop of the other. The IW was attempting to maneuver the top bundle off onto a pallet while doing so the stacked bundles fell pushing her backward her head struck part of the forklift mast.
