What happened
The driver auto-tarped the enclosed container and then walked around to secure the load to the truck. Since the container was enclosed no trash was expected to be on top. However the tarp may have shifted some loose trash. As the driver walked around a bottle fell from the top of the container striking him in the face and chipping his tooth.
