Worker was driving with another worker at the direction of multiple supervisors to drop off switch medications at the home of a client who had discharged. The medication was sucessfully switched. Upon driving to return back to [REDACTED] the van was at a downhill descent in a residential street covered in black ice and worker lost all brake power. Down the hill was an unrelated car accident and worker had to attempt to steer to avoid collision. This resulted in the van colliding with a tree.
