Driver [REDACTED] was attempting to get into the back of his box truck and as he was pulling himself up into the truck his right hand slipped and the driver fell to the ground from the deck of the back of his truck landing on his right side. The driver contacted me after work on [REDACTED] telling me of this incident. The Team member contacted me [REDACTED] stating that he has pain on his right side due to [REDACTED].
