Before the incident
Installing Rigid Discharge Electrodes (RDE) in the Electrostatic Precipitator (ESP)
What happened
Rigid Discharge Electrodes (RDE) had been removed from the RDE transport frame. The employees were rigging the empty RDE Frame for removal from the CE Support Bracket Tube. The frame was being positioned by the crane with instructions from the Rigger Signalman. Injured employee was grasping the end of the spreader beam with his fingers draped over the edge as the RDE Frame was in motion. The employees fingers (distal ends) were caught between the RDE Spreader Bar and CE Rectangular Support Tube causing amputation to the l 3rd fingertip and avulsions to the 2nd 4th and 5th distal fingertips. All motion was of the crane lift was stopped immediately. The injured employees hand was wrapped and he was transported by the Site [REDACTED] Coordinator to the Platte County Hospital Emergency Room.
