What happened
On [REDACTED] carrier called supervisor [REDACTED] saying that he had fallen and hit his head and was confused. Upon talking to the supervisor he stated many things that didn 't make sense and was unaware of where he was or what exactly happened only that he fell. An ambulance was called and upon the supervisor 's arrival the carrier was observed at [REDACTED] pacing back and forth at his vehicle. Shortly after the supervisor arrived the paramedics arrived and [REDACTED] was taken to the hospital the paramedics stated that he was very confused and stated that he has a small cut on the back of his head where he was injured.
