What happened
Employee 1st claimed that when he overthrew a sack and it ended up on the other side of the sack belt. When he went to retrieve it he tripped over the corner of the pallet of sacks then he showed me how he twisted his body (left side) and fell into the Birdcage that was near the pallet. He then insisted that I ask the witness to also reenact the scene literary walked over the pallet of sacks that were on the pallet that the employee said he tripped over the corner. he then stated he misremembered the events and that it did happen and the plastic around the orange sacks caught his left foot and he fell. He confirmed the eyewitness account. he also stated several times to me and SDO that he knew it was a safety violation and understands this was all his fault. He said he could use his right arm and hand to drive himself to ER. He originally told SDO he was fine at the time of the accident and then [REDACTED] he was in so much pain he came to the office himself to report to the MDO.
