Scanning MTE on the dock

On [REDACTED]. Clerk informed her supervisor that her back and hip were hurting her and she needed to go home. Supervisor asked her several time if it was a work-related injury. Clerk stated she had personal health issues that had caused for hip surgery and back issues that were not work related. On [REDACTED] Clerk requested light duty for her none work related injury. When informed that Managment needed documentation on her light duty restrictions she stated it was work related. Employee was scanning gaylords on the dock and while twisting to scan placards on gaylord 's she heard a pop in her lower back and hip area. Employee failed to report work related injury in a timely manner. Employee stated the injury was work related once management informed her that her light duty form needed to be submitted and based off her light duty restrictions work needed to be found.
