What happened
On [REDACTED] on dock at [REDACTED] while unloading an inbound trailer mail was loaded double stacked. On top of the double stacked mail there were multiple flat tubs with HAZMAT in it (Pictures attached). trip number was 002JU-62 from [REDACTED]. while driver was pulling out the mail from the door those flat tubs fell down on her shoulder and on the head of the Platform operator who was unloading the mail. and I asked both of them if they needed medical assistance but the platform operator (Name is [REDACTED]) denied assistance while employee requested medical assistance. But according to [REDACTED] the expeditor was distracted by using cell phone when he was unloading the mail which made her unaware of the surrounding and she could not see the flat tub falling on her.
