What happened
On [REDACTED] at approximately [REDACTED] near [REDACTED] on the platform an incident occurred involving Regular mail handler who fell from the rider jack he was operating. Upon management arrival at the scene management observed another mail handler 2 assisting mail handler 1 although mail handler 2 declined to provide a witness statement. Mail handler 1 fell to the ground impacting the back of his head and right shoulder. An investigative interview has been conducted to ascertain the root cause particularly as this marks the second occurrence of a similar accident for mail hander 1.
