Employee stated after [REDACTED] after coming out of the ladies restroom I stepped out into the lane and was hit with the back wheel of the forklift driven by [REDACTED]. I screamed for her to pull forward because my left foot was struck under her back wheel. She immediately pulled forward. There was no warning signal to indicate she was there. No horn was blowing.
