Employee astates that on [REDACTED] at around [REDACTED] as he was walking to the men 's bathroom he states he felt a tightness in his lower back as he was walking. Mr. [REDACTED] states the tightness went right away so he continued to walk but as he took a few more steps he his back locked up and he felt an extreme amount of pain in his lower back. Mr. [REDACTED] states that the pain was so severe that he could barely move. Mr. [REDACTED] states that he saw a coworker and asked to get him a U-cart so he could lean on it to walk. Mr. [REDACTED] states he used the U- cart to walk to find me ([REDACTED]) but ran into MDO [REDACTED] and told him that he hurt his back and needed to leave so he could take himself to the emergency room.
