What happened
Manager tripped over a crease in the ergo mat while she was sweeping the DBCS machine. The pie carts were moved and pushed over the mats causing the crease. This caused her to fall and resulting her hitting her left knee on the hard floor. The mats were not defective or curled up. Proper footwear was worn just failed to pick up her feet.
