What happened
Verified by SJ need statements and photos added. On [REDACTED] [REDACTED] at [REDACTED] PSE CC a PSE was sitting on console 1 when a carrier cell crash took place at the Bundle sorter. The crash damaged 2 carrier cells and broke the safety plexiglass. Employee left work early due to abnormal pain she also let me know that evening she was fine. On [REDACTED] [REDACTED] at [REDACTED] she contacted me that she was grazed on the chest.
