CALLED employee TO ASK FOR STATEMNT VIA PHONE [REDACTED]; ASKED Employee [REDACTED] FOR STATEMNT WHEN HE DROPPED OFF DOCTORS NOTE TO RETURN [REDACTED]. -MGMT AROUND [REDACTED] [REDACTED] WHILE DOING PIECE ON ROUTE 30 I WAS DELIVERING TO [REDACTED] WHEN I SLIPPED ON ICE ON SIDEWALK LANDING ON MY BACK AND STRIKING THE BACK OF MY HEAD ON PAVEMENT. AFTER BEING UNCONSCIOUS I AWOKE ON SIDEWALK AND WAS UNABLE TO GET UP. AFTER LAYING ON SIDEWALK FOR ABOUT 15 MINUTES OR SO A COUPLE DRIVING BY SPOTTED ME STOPPED TO OFFER AID AND CALLED [REDACTED]. A POLICE OFFICER [REDACTED] ARRIVED AND SHORTLY AFTER THAT AN AMBULANCE ARRIVED. -Employee ([REDACTED] STATMENT RECEIVED) ON [REDACTED] I ASKED Employee TO SHOW ME WHERE IN BACK OF HIS HEAD DID HE HIT HIMSELF AND HE TOLD ME HE HIT HIS HEAD ON THE RIGHT SIDE OF HIS EAR. NOT BACK -Supervisor
