What happened
[REDACTED] informed me that equipment operator and the general expeditor were involved in an accident and to obtain statements from both employees and pull the driving license for equipment operator. This occurred on [REDACTED] at approximately [REDACTED]. At the time [REDACTED] asked the expeditor if she was hurt and she stated she felt fine at the moment but the forklift had run over her foot however her foot was feeling fine at the moment of occurrence. She also stated at the time when the accident occurred it knocked her over when she was struck by the forklift and she landed on the floor. I then proceeded to pull the equipment operator off the forklift and obtained statements from both employees.
