What happened
While walking into [REDACTED] on the corner of [REDACTED] a customer opened the door for carrier. She passed by stepped on the wet dry carpet and slid in dampness on the right side of the carpet. She rolled her ankle inwards fell and had to be helped to stand up. The carrier did not notify me until she returned to the office. She did report it from her truck at the time of the occurrence. I told her to return to the office and let her know she would need to fill out a [REDACTED]. We can no longer do it. She stated she could not figure it out. She also stated today when I informed her I could not complete this without a doctor 's note that she would talk to the other supervisor. I told her until I had medical documentation I could not report this as medical.
