Before the incident
Retrieving items from the supervisor desk
What happened
On [REDACTED] at approximately [REDACTED]. I was retrieving an item out of the desk for Supervisor. This situation occurred in the old maintenance office. As I went to back up from the area there was a large dolly with pretty large wheels behind me. The back of my foot caught the wheel and I lost my footing. I fell backwards landing on my right hip arm and hand. another employee helped me to my feet and asked if I was ok at the time I did not notice if I was injured. I informed A OIC that I had fallen. As the night progressed my arm and hand started to hurt. I notified my A Manager the following morning [REDACTED] that I had fallen and wanted to file an accident report. I also told A OIC.
