CARRIER CALLED OFFICE TO REPORT INJURY. CARRIER RETURNED TO OFFICE AND REQUESTED MEDICAL ATTENTION. CARRIER WAS PROVIDED WITH CA-1 AND CA-16. CARRIER TOOK HER OWN VEHICLE TO DOCTOR 'S OFFICE
