Custodian fell on [REDACTED] while cleaning. She was on her way back to the custodial closet when she tripped over a folded-up mat that was placed next to the custodial door by another employee. Custodian fell hitting her head on the doorframe injuring her head neck and left hand. Custodian did seek medical attention.
