Employee was coming down the slide [REDACTED] after repairing an air leak. When he reached the bottom of the slide he stepped on a piece of cardboard not knowing there was a 18x18 hole underneath. his leg fell in hitting the steel frame and while falling in he hurt the other leg also hitting the steel frame.
