What happened

[REDACTED] Custodian came up to me outside the SMO office to let me know that she hit a divit in the pavement outside of [REDACTED] of the [REDACTED]. Custodian was driving the John Deere tractor that has the bucket on the front. She was driving the john deere tractor from the [REDACTED] door 8 towards the open area where the trucks travel to and from the dock doors with the bucket down to push the snow. During this process the bucket of the tractor hit an elevated portion of the pavement (divit) which was covered with snow. The impact caused her to lunge forward she ended up hitting her head on the center area just above the window. She told me she was wearing her seat belt moving slow and that she was all right but that her neck would probably be sore [REDACTED]. She decided to go to Penn Medicine Lancaster General Health; [REDACTED] [REDACTED]. She returned to work @ [REDACTED] or so with an Occupational Medicine Employee Medical Report.
