Manager received a supervisor portion of a CA-1 from DOL on [REDACTED]. The DOL Cause of injury states Work 12 hr days paid for 8 [REDACTED] changed my days off exp my FMLA for my child & how the change would effect her appointments she didn 't care immediately got a headache starting sweating crying felt my pressure rising I stepped out returned back she came antagonizing me about another supervisor felt my pressure rising vison blurred went back to work she returned again hostile my headache worsen got lightheaded felt like passing out put in leave said stress [REDACTED] later she called harassing me
