Moving equipment

I was attempting to move one of the APC that was full to the trailer when the top gate suddenly opened and hit me in the head. Although it did not knock me out it did push me back a little due to the impact and surprise of it happening. Coworker happened to be looking my way when it occurred. She asked if I was okay and came over to check on me. Although I was aware of what had happened I wasn’t sure what to do. Coworker sat me down and looked at the area where I was hit. Coworker was walking by and saw her tending to me and asked what happened. She told him and he asked if I wanted to write an accident report. Because there was no noticeable bruise or blood I initially said no. But then I started getting a really bad headache and felt a little dizzy. He went to get me an ice pack and I sat down while She gave me some Advil then went to get you.
