What happened
Employee assisted a fellow material handler with a bundle of cardboard that had fallen over. The other driver was setting the bundle back up and the bottom of the bundle was stuck. Employee pulled the bundle by the straps the strap on the right side broke and employee fell backwards striking head and tailbone on ground.
