What happened
Employee was traveling on ATV downhill at a low speed straddling a ditch. A tire slid into the ditch and employee was thrown from the ATV landing on tailbone.

Upon investigating the incident employee and company determined one of the back tires of the employee 's ATV was flat. A stick or debris caused the flat tire mid-trip which then pulled employee into the ditch.
